What is FertilityAssist 2

FertiltyAssist 2 is a new program designed to help cash-paying patients who may require a second treatment cycle. FertilityAssist 2 can make your treatment more affordable with savings up to $500* on Gonal-f®(follitropin alfa for injection) when you fill your prescriptions through Freedom Fertility Pharmacy.

How do I enroll in FertilityAssist 2?

Ask your healthcare provider to enroll you and fill your Gonal-f® prescription through Freedom Fertility Pharmacy. Download the FertilityAssist 2 Program Overview for reference when you talk to your healthcare provider.

*Important Eligibility Information

Fill your cash prescription for Gonal-f® through Freedom Fertility Pharmacy for two cycles of treatment.

  • First cycle must contain a minimum of 1050 IU
  • Second cycle must contain a minimum of 1050 IU for a cash savings of $250 or a minimum of 2100 IU for a cash savings of $500
  • Enroll in FertilityAssist 2 between April 1 - November 30, 2008
  • Second prescription must be received and shipped by Freedom Fertility Pharmacy on or before February 28, 2009
  • Only one card may be redeemed at a time and cannot be used in conjunction with any other offer or discount program

Important Safety Information

Gonal-f® should only be prescribed by doctors specializing in infertility or reproductive health. Gonal-f® can cause serious side effects including: ovarian hyperstimulation syndrome (OHSS), with or without blood vessel and lung complications and multiple births. Gonal-f® should not be used in patients who are allergic to recombinant FSH products, have primary gonadal failure, have uncontrolled thyroid or adrenal problems, have cancer of the sex organs, have a pituitary tumor or other tumor in their brain, have abnormal bleeding from the uterus or vagina or are pregnant or think they may be pregnant. The most common side effects in patients taking Gonal-f® include headache, upset stomach, breast discomfort and injection discomfort.
